If you have received services from any of our thirteen contract agencies, you will receive a notice of our privacy practices. The Health Insurance Portability and Accountability Act of 1996 (HIPAA) requires us to:
- Assure health information that identifies you is kept private
- Give you Notice of our legal duties and privacy practices with respect to health information about you
- Follow the terms of the Notice that is currently in effect
This notice basically describes how medical information about you may be used and disclosed and how you can get access to this information. You are not required to respond to this notice.
